Aim Statement Worksheet
Builds an aim with a population, a measure, a baseline and a date in it. Written to the IHI Model for Improvement format.
What's inside:

- Space for the version you will actually take to your QI lead

PDSA Cycle Sheet
One cycle per page, with the prediction written down before the test runs. Recording the prediction is what makes Study worth doing.

- Do, including what actually happened rather than what should have
- Study, comparing result against prediction
- Act, with the decision rule agreed before the data arrives

Driver Diagram Worksheet
Aim, primary drivers, secondary drivers and change ideas, laid out so the logic can be checked backwards from the change.

QIA Preparation Sheet
What to gather before a Quality Impact Assessment: the seven categories, likelihood and consequence, and the approvals that will be asked for.

SWOT Worksheet
The four boxes, with prompts that stop each one filling up with the same vague sentence. Ends by forcing a decision out of it.
